Game-used patches taking the shape of “USA” and a basketball” adorn one side of the booklet, while a black and red game-used patch boasting a black ink signature by “His Airness” resides on the other side of the booklet. A statement on the item’s back by Upper Deck attests to the authenticity of the signature and memorabilia components, and serves as the item’s COA. The limited-edition piece is serial-numbered “1/1.”
