Presented is former Utah Jazz head coach and former NBA player Jerry Sloan’s personal collection of awards, contracts and other items from his coaching career. The first item in this collection is Sloan’s “IBM/NBA Coach of the Month December 1993 Jerry Sloan Utah Jazz” award in a framed collage. The legend is engraved in black on a 1½ x 3½-inch gold metal plaque mounted at the lower center of a white with brown accent matte, below a 6½ x 9½-inch color photograph of Sloan in a suit and tie on the Jazz sidelines. The plaque and photo are displayed in a 12 x 16½ -inch brown wood with glass frame. The second item in this collection is a 14 x 17-inch laminated brown wood plaque with gold accents featuring a 11 x 13-inch color photo of Sloan coaching on the Utah Jazz sidelines over a 1 ½ x 4-inch gold metal plaque engraved with the caption “IBM/NBA Coach of the Month Award March 1997 Jerry Sloan Utah Jazz”. The third item in this collection is a 12 x 15-inch brown wood plaque with a 2 x 5-inch black metal plaque engraved in gold with the legend “IBM Coach of the Month March 2000 Jerry Sloan Utah Jazz” mounted beneath a 9 x 11-inch color action photo of Sloan coaching on the sidelines of Jazz game.

The fourth item in this collection is a 9½ x 14-inch laminated brown wood with gold accents plaque with a 1 x 3-inch gold metal plaque mounted at the lower center, engraved in black with the caption “IBM Coach of the Month March 1998 Jerry Sloan Utah Jazz” mounted below a 6½ x 9½-inch color photo of Sloan on the sidelines. The fifth item in this collection is a 11 x 15-inch laminated brown wood with gold accents plaque with a 8 x 10-inch color photo of Sloan coaching the Jazz at the top center, and a 1¼ x 3¼-inch gold metal plaque mounted at the bottom center, engraved in black with the caption “IBM Coach of the Month February 1999 Jerry Sloan Utah Jazz”. This collection also includes correspondence from the University of Evansville, and a 9 x 12-inch brown wood plaque is engraved in black with the seal of the university, the 150th Anniversary logo, and the legend “University of Evansville Alumni Association 2005 Distinguished Alumnus Award presented to Jerry Sloan ’65 M ’73 for distinguished achievement in service to his community and exceptional leadership and service to the University of Evansville”. This collection also includes an 8 x 10-inch brown wood plaque with a 7 x 9-inch silver metal plaque engraved in black and red with the 20th Season Chicago Bulls logo at the top center and the caption “Presented to Jerry Sloan in appreciation for your participation in the Chicago Bulls 20th Anniversary Reunion Game December 21, 1985” with the logos of United (airlines) and Schick in the lower center.

Sloan’s collection also includes a 7 x 9-inch brown wood plaque with a 5½ x 7¼-inch gold metal plaque engraved in black with the outline of the state of Illinois and the caption “Illinois Basketball Coaches Association Hall of Fame Jerry Sloan Player McLeansboro For Outstanding Achievement in Illinois Basketball”. This collection includes a 2½ x 4 x 6-inch crystal trophy etched in white with the legend “GMC Sporting News NBA Coach of the Year 2003-2004 Jerry Sloan Utah Jazz” in the original padded gift box, as well as a 2 x 4½ x 7-inch Lucite “National Basketball Coaches Association NBA Western Conference Coach of the Month November 2006” trophy. Also included in this collection are documents, including a December 12, 2012 letter to Sloan on Illinois High School Association letterhead congratulating him for having the McLeansboro High School Gymnasium named after him. The collection of documents includes a binder labelled “CONFIDENTIAL! Utah Jazz 1998-99 Regular Season NBA Player Statistics and Rankings Prepared May 14, 1999,” miscellaneous Chicago Bulls contracts and correspondence to the former NBA star player, legal and financial documents and statements, a proclamation in honor of Sloan from the State of Illinois House of Representatives 97th General Assembly in 2012, and miscellaneous Utah Jazz contracts and correspondence to the longtime Jazz head coach.  Sloan played 11 seasons in the NBA primarily with the Bulls before beginning a 30-year coaching career, 23 of which were spent as head coach of the Jazz (1988–2011).
