Presented is a dual-signed Marilyn Monroe and Arthur Miller Associated Hospital Service of New York hospital service plan. The couple’s signatures are in blue pen on the interior third page of the four-page yellow paper document, with Miller listed as the husband. The legendary Hollywood film star has signed her name “Marilyn Monroe Miller” below her husband. What’s significant about this document is on page 2 Monroe disclosing her past pregnancy and having her appendix removed. Under the (b) subheading of “Have you within the last five years:” (b) had an operation performed on you or been a patient in a hospital or similar institution?” “Yes” is handwritten with “(B) Ectopic Pregnancy” written for the explanation. In addition, under the question “When did you last consult a physician? Give date, name and address of physician, and particulars”, “Dr. Bernard Berghas, Relative to miscarriage” was written as an answer. The 8½ x 11-inch document is folded in thirds and has inscriptions on pages two and three in blue pen. This document is accompanied by an 8½ x 11-inch black and white photograph of the couple. On June 29, 1956 Monroe and Pulitzer Prize-winning playwright Arthur Miller were married at the Westchester County Court in White Plains, New York. The marriage, Monroe’s second (after Joe DiMaggio) ended in 1961. This item comes with a full LOA from PSA/DNA (AH04085).
