Presented is a questionnaire filled out by baseball great Pete Rose. Known as “Charlie Hustle”, Rose is the all-time MLB leader in hits (4,256), games played (3,562), at-bats (14,053), singles (3,215), and outs (10,328). Rose is truly one of the best players in MLB history and his on-field abilities can never be questioned. This featured questionnaire was filled out by Rose himself in bold blue ink. It is a front and back sheet, issued by William J. Weiss and dated from July 19, 1960, while he played for the Cincinnati Reds. Interestingly enough, the questionnaire was filled out before Rose had ever married or had any children! It reveals things such as his personal nickname “scooter”, as well as what sports he participated in high school, as well as his off season hobbies.
