The award incorporates a 11½ x 15-inch watercolor portrait of Sloan in a purple shirt, as well illustrations of Jazz superstars Karl Malone and John Stockton, the Delta Center arena, and the Larry H. Miller automobile dealership along with individuals representing the dealership and the associated charity. The print is mounted at the top center of a gray matte with navy-blue accent. Mounted below the print is a gold metal plaque with the caption “Presented to Jerry Sloan in Celebration of twenty years of service with the Larry H. Miller Group of Companies Beginning 11-29-1984” engraved in black type. The print and plaque are displayed in an 18½ x 22½-inch brown with black and gold accents with glass frame.
